* In the case of a minor, copy of the minor’s Birth Certificate and * a copy of the guardian’s Identity Document

* National Senior Certificate and/or copies of any post Matric * qualifications (including vocational studies)
* Copies of payslips and or proof of income:

I. Proof of income at the time of the reported accident
ii. Proof of current income
iii. Employer’s Certificate indicating losses suffered
iv. Employer’s Certificate confirming employment and / or

letter of confirmation from employer v. Benefits statement (where applicable)
